What is person centred care?
Aged care has shifted in focus from a hospital style care focussed on the priorities of staff, routines, time and tasks to a person-centred care that is created from the individual preferences and uniqueness of each person involved in the care. This includes the person being cared for as well as those caring, staff, family members, friends and management of the care environment.
Tom Kitwood with Kathleen Bredin, developed the Dementia Care Mapping (...more) tool to evaluate the quality of dementia care. This tool was introduced to Australia by Virginia Moore, under the auspices of the Brightwater Group in Perth, Western Australia.
